    The challenge was fragmented risk, with a patient’s glucose spike, doctor’s schedule, and family awareness existing in separate silos, leading to reactive rather than proactive care. The solution connected Patient and Practitioner apps through real-time risk data, automatically alerting the doctor, front desk, and family when risk increased. This enabled personalized monitoring, streamlined booking, and a gamified wellness loop, giving patients and practitioners one shared, real-time view of risk.
